Thank you for your participation in the Family Literacy Summer Grant Program.  Below you will find the online form to submit your final report on your organization’s participation in the program as a successful recipient.

Before you proceed:

• Ensure you have reviewed all the questions below prior to starting your application.  You will not be able to save your progress once you begin so it is important to have all your information on hand prior to.
• Final reporting is due before September 27th, 2024 by 11:59PM.
